Its good to run a diskcleanup first and then defragment. Or run a diskcleanup every few weeks and defrag can be automated using one of the real time defrag programs which not only run along with other programs, but also defrag under low free space.

I work in the Tech Support section of Diskeeper Corporation so I’ve had a lot of experience in dealing with fragmentation and the impact of fragmentation on machines. As indicated here, fragmentation is often responsible for greatly slowing performance, reducing the longevity of you hard disks and for other issues in reliability. We’ve done a lot of research into the effects of fragmentation and we have several white papers covering our findings if anyone is interested in fragmentation as it bears on a particular harddisk type or machine configuration.
